What is the GUIDE Program?

GUIDE stands for Guiding an Improved Dementia Experience. It’s a Medicare-supported program from the Centers for Medicare & Medicaid Services (CMS) designed to improve care for people living with dementia while giving family caregivers real, practical support — at no cost to eligible participants.

Instead of leaving families to piece together care on their own, GUIDE connects each participant with a dedicated care team and a clear plan for navigating what comes next.

What the GUIDE Program Offers

• A dedicated dementia care navigator who coordinates care and connects your family to community resources like transportation, meal support, and safety programs.
• Caregiver training and support to help you manage stress, improve day-to-day safety, and delay the need for a higher level of care.
• Free respite care, paid for by Medicare — eligible families may receive up to $2,500 per year toward in-home respite care, renewed annually, so your loved one can stay safely and comfortably at home while you get the break you need.

Who Qualifies for GUIDE?

You may be eligible if the person you’re caring for:

• Is enrolled in Original (Traditional) Medicare
• Has a suspected or confirmed dementia diagnosis (Alzheimer’s, Lewy body dementia, frontotemporal dementia, vascular dementia, and others)
• Lives at home or in the community, including assisted living or independent living

GUIDE is not available to those who:

• Are enrolled in a Medicare Special Needs Plan (SNP) or Medicare Advantage
• Live in a long-term care nursing home
• Are enrolled in Hospice or PACE

Eligibility rules are set by CMS and can be updated, so the best way to know for sure is to give us a call.
